I understand that I may be asked to submit to a drug test and subject to disciplinary action if the requirements set forth by the Policy statement are not met, as a result of a positive drug test. I understand that AHC will not supply personally identifiable test data to law enforcement agency. *
I understand that I may be removed from the team if I fail to comply with the standards set forth by the Athletic Department, the Coach, the Conference/State Constitution and Bylaws, or College Personnel *
I understand that I may be assess fees, which may impact my registration, if I do not return my equipment according the AHC policy. *
I understand I must complete the sexual violence training before the first date of competition. *
